Service Description: Provides children and youth with multiple and/or complex special needs and their families with a co-ordinated service planning experience by:
  • Linking families to other services and ensuring that all professionals involved are working together as a team
  • Knowing that providers from different sectors will be communicating about the needs and goals of the family and keeping the family informed at all times

Families who would benefit from Coordinated Service Planning include:
  • Having a child or youth with complex special needs up to 21 years of age who is still attending school
  • Receiving or waiting for services from more than one service provider (including schools, therapist, community supports, and medical professionals)
  • Finding it difficult to navigate and coordinate multiple services and; want to ensure the best plan of care for their child or youth

  • Service providers, including physicians can submit a general referral form by visiting the Get Services Now Page here
