Service Description: Promotes the rights and well-being of individuals with Alzheimer's Disease and their caregivers

Support offered by phone during set hours by First Link Care Navigator
  • Support for anyone impacted by dementia, at any stage, in every step of their journey
  • Personalized direction to help navigate dementia-related services, resources and next steps
  • Accessible support with no referrals or waitlists

  • Individuals with Alzheimer's disease or other dementias

  • Family, friends, and other partners caring for a person with dementia

  • Individuals concerned about memory loss or other cognitive changes
