Service Description:
|
Family Health Team includes: Family Physician, Registered Nurses, Registered Practical Nurse, Social Worker, Registered Dietitian, Addiction Program Care Coordinator, Administration and other professionals. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Provides medication and therapeutic treatment for opiate addictionBased on an intensive outpatient modelOffers initial intake screening, appropriate lab tests, regular medical follow-ups and regular suboxone dosing

- Open to referrals from health care provider or self-referral
- New patients must complete intake process before becoming a registered client of the clinic
