Provides supportive housing and community support services for seniors and community members with disabilities
Partners with Dryden Native Friendship Centre to provide transportation servicesProvides transportation to medical appointments, shopping, and social activitiesWorks by providing up to 6 discount taxi vouchers per month

- Initial assessment required
- Requires completion of a photo identification card (valid for 1 year); done during initial visit
|
|
Eligibility / Target Population:
|
Must be 60 years of age or older and meet one of the following criteria:
- The client is in receipt of or eligible to receive other long term care community services
- The client is isolated and requires social interaction or stimulation
Transportation
Client must meet one of the following criteria:
- Unable to access public transportation because of short or long term physical limitations
- Requires escort during the trip because of frailty or cognitive impairment
- Specialized transit is not available or the individual does not meet the eligibility criteria for Handi-Transit
- Public transportation is not available and friends or relatives are not available or able to provide transportation
