Service Description: The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Medical care team focused on family and community health
  • Offers timely access to high quality primary health care
  • Coordinates documentation if patient receives care from a member of the team
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Administers various health programs
Provides health care services including but not limited to:
  • Chronic disease management
  • Diabetes program
  • Health promotion
  • Disease prevention
  • Nutritional counselling
  • Mental health clinic
Telephone Health Advisory Service
  • Provides access to a Registered Nurse who provides advice about urgent health care concerns
  • Offers the opportunity to have family doctor receive a written summary of the call

Fees: None - Most services covered by OHIP
Application: Book appointment online or call office
Eligibility / Target Population:
  • Must be a patient of a Sunset Country Family Health Network physician at the Paterson Medical Centre, Keewatin Medical Clinic or Lakeside Medical Office
  • Must have valid OHIP card to receive services
