Service Description:

Medical care team focused on family and community health
Medical care team focused on family and community health
- Offers timely access to high quality primary care
- Provides access/referrals to other health care professionals within and outside of the Medical Associates and Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Provides follow-up by RN when discharged from hospital
- Provides an urgent care clinic
- Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)
Provides the following programs and services:
- Allergy shots
- Ear flushes
- Urgent care assessments
Offers primary health care including but not limited to:
- Chronic disease management
- Health promotion
- Disease prevention

- No referral required for most services

Eligibility / Target Population:
|
- Must be rostered with a Physician of the Medical Associates to book appointment
- Call Clinic to have name added to waiting list for rostering
