Service Description:
|
Family Health Team includes: Nurse Practitioners, Registered Nurse, Registered Social Worker, Health Educator, and Administration who work closely with Physicians in Geraldton. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.
Medical care team focused on family and community health- Offers timely access to primary care through Advanced Access (same-day bookings)
- Provides access to other health care professionals within and outside of the Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Administers various health programs
Offers primary health care services including but not limited to:- Chronic disease education and prevention
- Disease management
- Health promotion
- Palliative care

Walk-in Clinic
Present in-person between 8:30 am-2:30 pm to sign up and receive approximate time to return to see health provider - appointments are given on a first come-first served basis

- Registered clients of the Family Health Team
- Must have a valid Ontario health (OHIP) card
