Service Description:
|
Operates a self government and is responsible for the day to day operations of the Pays Plat First Nation- Tribal Council: Unaffiliated
- Political Territorial Organization: Union of
Ontario Indians (UOI)Robinson-Superior Treaty
Provides services that promote physical and mental health for community members- Coordinates health care services through a health team which consists of Community Health Representative, Community Wellness Worker and a Family Support Worker, visiting Physicians and Crisis Team workers
- Provides health awareness and counselling, education and prevention workshops/clinics in areas that may include Child Care, Diabetes, Elderly & Chronic Disease, Immunization, Sexual Health and Communicable Diseases
- Coordinates basic foot care clinics
- Identifies and attempts to correct health hazards
Addresses individual health care needs by providing:- Intake services to determine health care needs of individual
- Treatment of on-going health problems
- Referrals to appropriate health care providers based on information in the intake assessment
- Advocacy on behalf of individual with other health professionals
- Coordination of one-on-one pre-natal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
