Service Description: Supports for people living with pre-diabetes or diabetes
  • Provides a Certified Diabetes Educator (RN) and a Registered dietitian to help individuals manage their diabetes
  • Assesses, monitors, and assists individuals with diabetes management and to attend to any diabetes complications they may have
  • Offers education and self-management support to fit an individual's lifestyle

Offers the following interventions:
  • Complication/Co-morbid condition management
  • Insulin starts and adjustments
  • Healthy Lifestyle
  • Meal Planning
  • Nutritional assessments
  • Pregnancy management
  • Type 1 and Type 2 management

Application: Call * no referral required
Eligibility - Population(s) Served: Aboriginal, Métis, and Non Status individuals with pre-diabetes, type 1 and type 2 diabetes, or gestational and pre-gestational diabetes
