Service Description: The Atikokan Family Health Team includes: Physicians, Registered Nurses, Registered Practical Nurses, Dietitian, Mental Health Worker, Pharmacist and Administration

Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy. The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues

One of two sites - located on the lower level of the Atikokan Medical Associates Clinic

Medical care team focused on family and community health
  • Offers timely access to high quality primary care from a Clinic site and an Outreach site
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Provides follow-up by RN when discharged from hospital
  • Provides an urgent care clinic
  • Offers the Standford Chronic Disease Self-Management program
  • Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)

Provides the following programs and services:
  • Allergy shots
  • Breast Start Program
  • Chronic Obstructive Pulmonary Disease (COPD) Program
  • Ear flushes
  • INR Program
  • Smoking Cessation Program
  • Teledermatology
  • Urgent care assessments

Offers primary health care including but not limited to:
  • Chronic disease management
  • Health promotion
  • Disease prevention

Fees: Most services covered by OHIP
Application: Call for appointment * no referral required for most services
