Service Description: Offers timely access to high quality primary care

Health Clinic building is located at Anne St near Pickle Lake Rd and includes dental office, Best Start Hub, fitness centre and office for visiting health providers

  • Offers timely access to high quality primary care
  • Provides access to other health care professionals outside of the clinic
  • Coordinates documentation if patient receives care from a member of the team
  • Provides diabetes case management
  • Provides blood pressure checks
  • Offers mental health counselling through a visiting mental health counsellor from Sioux Lookout on a regular basis

  • Offers primary health care including but not limited to:
    • Chronic disease management
    • Health promotion
    • Disease prevention
    • Nutritional counselling

    Fees: Service - Some tests and/or forms are not covered by Ontario Health Insurance Plan (OHIP) * None
    Application:
    • Recommends booked appointments be made in advance
    • Accepts walk-ins if time allows during the two weeks that a doctor is in attendance
